浅析鼻敏水治疗变应性鼻炎大鼠的实验探讨

摘要:目的探讨鼻敏水滴鼻剂治疗变应性鼻炎药效。方法以卵清白蛋白致敏大鼠制做I型变态反应动物模型,随机分为3组:模型对照组、鼻敏水组、药物对照组,并设正常对照组,分别观察各组大鼠行为学表现及测定其鼻黏膜组胺含量、鼻分泌物和血清嗜酸性粒细胞、血清总IgE水平。结果行为学积分,给药后鼻敏水组、药物对照组行为学积分明显低于模型对照组(P<0.01),鼻敏水组积分低于药物对照组(P<0.01);鼻敏水组和药物对照组鼻黏膜含量、鼻分泌物嗜酸性粒细胞明显低于模型对照组(P<0.01),鼻敏水组和药物对照组比较有显著性差异(P<0.01);鼻敏水组和药物对照组血清嗜酸性粒细胞记数、总IgE水平明显低于模型对照组(P<0.01),鼻敏水组和药物对照组比较无统计学意义(P>0.05)。结论 鼻敏水滴鼻剂能降低实验性大鼠鼻黏膜组胺含量,抑制或降低其血清总IgE水平及嗜酸性粒细胞记数,提示鼻敏水滴鼻剂对变应性鼻炎具有治疗作用。

Experimental Research on the Treatment with BiMinShui on Allergic Rhinitis of Rats
Abstract:ObjectiveTo investigate the medical effect of BiMinShui nose drops on the treatment of allergic rhinitis. MethodsTo make typeⅠallergological rat models allergized by ovalbumin, and divide the models into three groups at random:the group of control, the group of BiMinShui and the group of drug control.Besides, to establish a group of nonmedical models. To observe the ethological performance of rats and examine the contents of histamine in nasal nucous coat, nasal secretions; oxyphilic granular leukocytes and total IgE values in blood serum.ResultsThe posttreatment ethological integration in the group of BiMinShui and the group of drug control was significantly lower than that in the group of control(P<0.01).Integration in the group of BiMinShui was lower than that in the group of drug control(P<0.01).The contents of histamine in nasal mucous coat, nasal secretions and oxyphilic granular leukocytes in the group BiMinShui were obviously lower than those in the control group(P<0.01).The distinction between the group of BiMinShui and the group of drug control was significant(P<0.01).The counting of oxyphilic granular leukocytes and total IgE values in the group of BiMinShui and the group of drug control were evidently lower than those in the group of control(P<0.01),but the distinction between the group of BiMinShui and the group of drug control was of no statistical significance(P>0.05).ConclusionBiminshui nose drops can reduce the contents of histamine in nasal nucous coat of experimental rats, constrict or decline the total IgE values in blood serum and the counting of oxyphilic granular leukocytes. BiMinShui nose drops is effective in treating allergic rhinitis.

变应性鼻炎 (AR)是IgE介导的I型变态反应性疾病,该病发病率有逐年上升趋势。据国外统计,发病率为10%~40%[1],在我国患病率为37.74%[2],每年约有2 000万人患病[3],Bousquet指出,必须将PAR看作严重的全球性健康问题。[4]现代通过动物模型观察鼻黏膜的改变,发现鼻黏膜有明显的变应性炎症反应,提示对变应性鼻炎治疗中鼻腔局部用药的重要性。本研究通过观察鼻敏水滴鼻剂对实验性变应性鼻炎大鼠鼻部症状、鼻分泌物和血清中嗜酸性细胞、鼻黏膜组织形态、鼻黏膜组胺含量、血清总IgE的影响,以探讨其对PAR的防治作用,为临床应用提供理论依据。
1 材料与方法
1.1 材料
1.1.1 实验动物 清洁级Wistar大鼠40只,雌雄各半,体重(160±50) g。由广西中医学院实验动物中心提供。
1.1.2 药品 鼻敏水滴鼻剂药物组成:辛夷花、白芷、百部、牛蒡子、蒺藜、鱼腥草、地肤子、鹅不食草、荆芥、薄荷等。由广西中医学院第一附属医院制剂室提供,10 ml/支,批准文号:桂卫药制字(1998)005025。0.5%麻黄素地塞米松液(1%麻黄素滴鼻液5 ml+地塞米松注射液2.5 mg+生理盐水5 ml,由广西中医学院第一附属医院制剂室配制提供)。
1.2 方法
1.2.1 动物造模、分组及给药 清洁级Wistar大鼠40只,完全随机分为4组,除正常对照组(10只)外,其余各组按林健、曹平等[5]报道的方法制备变应性鼻炎动物模型。①每只大鼠分别予腹腔注射卵清蛋白混悬液(卵清蛋白0.3 mg与氢氧化铝30 mg溶于1 ml生理盐水配成),同时给正常对照组每只注射生理盐水1 ml,隔日注射1次,共注射7次。②腹腔注射完成以后第2天,配制0.5%的卵清蛋白生理盐水溶浓,经雾化后导入一只侧面开洞的大鼠盒中,将需要造模的大鼠每批1笼(10只)放入大鼠盒中,上面用洁净的玻璃盖好。开启雾化器,调节汽雾流量,每批10 min左右,保证每批雾化量要达到10 ml,平均每只豚鼠吸入2 ml左右,正常对照组用纯生理盐水同样操作。每日吸入一次,共连续5次。③隔3 d后,取2%卵清蛋白生理盐水液,通过滴管对造模组大鼠逐只进行滴鼻,平均每只大鼠每侧鼻孔滴入0 .1 ml; 正常对照组组用生理盐水同样操作。1次/d,共滴4次。每次鼻腔致敏后,立即观察30 min。记录大鼠鼻分泌量、喷嚏及搔鼻次数,采用叠加量化记分法记分。参考2004年兰州会议制定的变应性鼻炎的诊治原则,拟鼻痒:无抓鼻为0分,轻度抓鼻为1分,频繁抓鼻为2分,抓鼻不止为3分;喷嚏:无喷嚏0分,打喷嚏1~3个为1分, 4~10个为2分,大于11个为3分;鼻涕:无鼻涕为0分,流至前鼻孔为1分,超出前鼻孔为2分,涕流满面为3分。总分超过6分者示造模成功。模型判定成功后隔日滴入相同浓度的卵蛋白溶液,以加强致敏,正常对照组用生理盐水代替,方法同上。自造模成功第2天起,正常对照组及模型组用生理盐水滴双侧鼻孔,每侧10 μl,2次/d;鼻敏水组(治疗组)用鼻敏水滴鼻剂滴双侧鼻孔,每侧10 μl,2次/d;药物对照组用0.5%麻黄素地塞米松滴鼻剂滴双侧鼻孔,每侧10 μl,2次/d。连续用药10 d。
1.2 观察指标及方法
1.2.1 鼻部症状观察 采用症状积分叠加评分方法(见模型判定),比较组间差异。
